You must be consulted and involved, and your carer also has a right to be consulted.

When assessing what help you'll need to live independently in the community, the Social Work Department mustn't take into account how much it might cost.

They should consult with the local NHS Trust or Hospital, Housing Department and others as well if necessary to work out what help you need and how it might best be provided.

You might need home support and day care, assistance in the home, or supported employment or training, support groups, etc. You might need help all day and all night, or throughout the day, or just part of the day.

You should be given a copy of the assessment of your needs if you ask for one; at the very least, you ought to be told about the outcome of your assessment and you have a right to ask to see your Social Work file.

If you see how the Social Worker has assessed your needs and you think you need help that hasn't been listed in the assessment, or you think something's been missed out, this is an excellent chance to put your views forward.
