Exercise Referral Scheme

The Live Active Exercise Referral Scheme is designed to assist those, who could benefit most, to take their first step towards being more active and to support them to live actively.

Am I regularly physically active?

Every Adult should aim to accumulate at least 30 minutes of moderate intensity physical activity over most days of the week.

This should raise your heart rate enough to make you feel warm and give you a faster rate of breathing. At this intensity you should still be able to talk without being too breathless.

Why be active?

There is now extensive evidence, which shows that an active lifestyle will help you:

  • Feel more energetic and lively

  • Improve your health

  • Feel better

  • Control your weight

Is the exercise referral scheme for me?

The good news is that everyone can enjoy these and many more benefits!

Making small changes to your daily routine, such as taking the stairs instead of the lift, walking to the shops or getting off the bus a stop earlier can make all the difference!

The Scheme may be appropriate for you if…

  • You are not currently regularly physically active but you are thinking about becoming more active

  • You are doing some activity but would like to do more and fell additional advice and support would help you

  • You have been advised to become more active e.g. by your GP or Practice Nurse but you don’t really know where to start

How will the scheme help me become active?

Once referred, you will be invited to attend an exercise consultation in your local area with an exercise counselor. You will enroll onto the scheme for 12 months where you will-

  • receive advice, support and encouragement from an exercise counselor to help you become and stay regularly physically active

  • have an opportunity to meet other people and try new activities

How can I access the exercise referral scheme?

If you are interested in the exercise referral scheme, contact your GP, Practice Nurse or Physiotherapist. If the scheme is appropriate for you, they will complete a referral form and the exercise counselor will contact you direct to discuss the background to your referral and follow through with an initial consultation.
